Literacy First Phonics Assessment
(2001) Literacy First Process. Professional Development Institute, Inc. Mill Creek, WA.
This assessment has three parts; letter names and sounds, reading and phonetic decoding and sight words. The sections are differentiated by grade level of phonics skills.  This assessment may be given along with the Phonological Awareness Skills Test (P.A.S.T.)
Woodcock-Johnson III Diagnostic Reading Battery 
Houghton Mifflin Harcourt (2004)
The Woodcock-Johnson III Diagnostic Reading Battery assessment is administered individually to determine strengths and weaknesses in reading.  Subtests include Brief Reading, Reading Comprehension, Phonics Knowledge and Oral Language Comprehension.  The Basic Reading subtest is a combination of the vocabulary and phonics.

Stanford Diagnostic Reading Test, Fourth Edition (SDRT)
Pearson (2004)
The Stanford Diagnostic Reading Test can be used to asses abilities in decoding, vocabulary, comprehension and scanning.  Subtests enable detailed diagnostic information about phonics skills.

Fox in a Box 
CTB/McGraw-Hill (2004)
The Fox in a Box assessment includes kits for phonological awareness, phonics, fluency, comprehension and vocabulary.  

Roswell-Chall Diagnostic Assessment for Reading (DAR-2)
Houghton Mifflin Court (1997)
The Diagnostic Reading Test measures letter recognition, phonics and other word recognition skills.  There are two forms of the assessment given in a pre- and posttest setting.  The DAR-2 helps teachers identify students who need help in reading, and diagnose those difficulties with reading.  

The Decoding Skills Test (DST)
Richardson & DiBenedetto (1985)
The Decoding Skills Test provides a profile used to diagnose decoding skills that are essential to reading comprehension.
